What insurance companies are you affiliated with?

We will provide you with a superbill so you can submit claims on your own.

Can you bill my insurance company for me?

No, but you can submit the bill to your insurance  company.

 

You can use your Health Care Spending Account (HCSA) or your flexible spending dollar to pay for services (HCFSA). A Health Care Spending Account (HCSA) is a tax-advantaged benefit that allows employees to set aside pre-tax dollars to pay for eligible medical, dental, and vision expenses. A Healthcare FSA (HCFSA) is a tax-advantaged account that allows employees to set aside pre-tax dollars to pay for qualified medical, dental, and vision expenses.

 

Lab Workups

Laboratory testing with Labcorp will be billed to your insurance company except for HMO. We can not bill HMO lab. HMO patients will be treated as cash patient.

Do you accept Medicaid or Medicare?

Unfortunately Medicare and Medicaid does not pay for our services so we can not bill them.

When is payment due?

Payment is due at the time of service. Your insurance company will reimburse you for what they deem appropriate.

What form of payment do you accept?

Cash, check, credit cards, debit cards, health care spending accounts.

Can I cancel an appointment?

Should you need to cancel an appointment, 24-hour notice is required. 24-hour notice of cancellation is $50.00 per 30 minutes. Leaving a message the night before the appointment is NOT a 24 hour cancellation.
